  • Essay / Critical Incident Analysis - 1642

Before the clinic started, I did my usual preparation for the following clinical days in week 3. I thought I was well prepared for the clinic, I knew the conditions mental and medical conditions of the client. But I was focusing more on the client's mental illness and not their illness, so when the nurse came to me and told me that Client W was suffering from shortness of breath, I was a little shocked because I didn't I didn't expect any unexpected events. Introduction to the Clinical SituationThe first day of my second clinical week was the first day I had two clients in one shift. Customer W and I were on our way to the dining room and another customer stopped me and asked me to put on his jacket, so I told my customer I would meet him in the dining room. After doing this I was on my way to the dining room and Nurse A said Client W was having difficulty breathing and we needed to give him his 0900 tampons. He had audible wheezing in his lungs and his respiratory rate was rapid. So we had to give the client three different inhalers, SalbutaMOL sulfate is a bronchodilator that allows the alveoli in the lung to open so we can administer the next two buffers, ipratropium to help reverse the obstructed airways respiratory tract due to his COPD and Advair diskus for his asthma. Client W felt much better after receiving these puffs, his respiratory rate was stable and he no longer wheezed like he did before receiving his puff medication. We suggested that he lie down for at least 30 to 60 minutes after administering the medication, to allow his lungs to rest. For some reason he was up and walking around the unit and experiencing wheezing, but he would turn better as soon as we took him back to his room and stayed next to the bed with him, his breathing became much better with the Fowler's position in bed. And then, after administering the 0900 meds, he asked for Tylenol for his "severe" headache which many nurses had told me was probably attention-seeking behavior. Because he would feel better as soon as you talked to him, took him back to his room and told him to lie down and do a deep breathing technique..
