This has been divided into 3 parts. Better to ensure that primary (community-based) and secondary (hospital-based) care work together professionally by sharing patient information. and ensuring that patients receive a good service (Department of Health, 2011). Previously, this was about reducing waiting time for patients by ensuring that the services provided to the community by their GPs (GPs) ran smoothly between different health services and also working as a team ( Ministry of Health, 2011). A more practical way of meeting patients' needs by sharing their information and working together to ensure their needs are supported and bringing more services closer to communities (Ministry of Health, 2011). The New Zealand initiative involved launching a number of community-based health and primary care systems, including GPs, nurses and other health groups (Ministry of Health, 2011). Alma Ata (1978) states that health centers should be closer to where people live and work, this is what HVAZ achieved by being able to go and assess the community where they will be socially comfortable. HVAZ nurses provided better, faster and more convenient services by going into the community and assessing their health concerns through their churches, a location closer to them.
