Lo, Stuenkel, and Rodriguez (2009) emphasize that organized and well-prepared discharge planning, patient education with multilingual services, and the use of different teaching methods significantly improve patient outcomes (p. 157). These include experienced and well-taught follow-up phone call sessions after discharge as well as ensuring adequate post-operative care continues. Another way nurses provide planned discharge teaching is to provide a straightforward checklist for the patient and family to follow.
